Prior to better medical care and access to medical care, you didn't hear about sick children because they just up and died instead.
Maybe some statistics will give you some perspective:
During the 20th century, it is estimated that smallpox was responsible for 300–500 million deaths. Over 80% of infected children died from the disease.
In the 1920s there were an estimated 100,000 to 200,000 cases of diphtheria per year in the United States, causing 13,000 to 15,000 deaths per year. Children represented a large majority of these cases and fatalities.
There are about one million cases of tetanus reported worldwide annually, causing an estimated 300,000 to 500,000 deaths each year. In the United States, approximately 100 people become infected with tetanus each year, and there are about five deaths from tetanus each year. Nearly all of the cases in the United States occur in unimmunized individuals or individuals who have allowed their inoculations to lapse.
Before vaccines, an average of 157 cases per 100,000 persons with Whopping Cough were reported in the U.S. More than 93% of reported cases occurred in children under 10 years of age.
